How to apply in Waupaca County

You may qualify for Medicaid if you are a citizen of the United States or an "eligible" person, meet the financial eligibility requirements, and are in one of the following categories:

• Age 65 or older.
• Blind.
• Disabled.

For Medicaid only (not applying for any other program at the same time), you may apply in person or by phone as described in the above link, or you may apply by mail.  Get a printable application form from the State of Wisconsin Medicaid website, complete the appropriate form, and mail to:
